We are located out of state. Do we need special license to transport water to AZ? Can we bid only on the locations that need 16.9 oz bottled water?

The license requirements are the responsibility of the prospective offeror. No, the entire solicitation and all requirements must be bid to.

Will the government please confirm the revised quantities? The current QTY says "12 MO" for all CLINs, but there are no actual units per month
Response Received:
The quantities listed are monthly requirements. The contract period of performance shall be 12 months. The units required per months are as listed. The case-lot requirements quantities indicate the number of cases (24 bottles (16.9 oz) or 6 bottle (gal)) cases. The only single bottle requirement listed is in the 5-gallon category.
Deliveries of the quantities listed shall be required in total per each month of the 12-month period of performance. The actual days for delivery of the monthly requirements shall be coordinated between the Contractor and the customer after award. But the entire monthly allotments as indicated on the line items shall be required each month. Discounts for volume should be considered accordingly.

Do you have an example of a "Business Health Certification" We are not familiar with this requirement in providing COTS potable water. Also, the "Water Handler Physical Examination" Certification is only required for manufacturer/operator of water bottling facilities. We are a small business non-manufacturer and are merely providing COTS water. We have provided the manufactures water test results with the previous bid, will this suffice?
Response Received:
Business Health Certificate refers to the business license required to do business as a bottled drinking water distributor or manufacturer, within the industry-standard FDA requirements. If you can provide information that your bottled water will be coming from an approved source, and will not be packaged, processed, or otherwise manufactured at your facility, a Water Handler Physical Examination certification is not required, and manufacturer test results will suffice.

Monthly required totals. Requirements are not estimates as the full amounts as listed will be utilized under this Firm Fixed Price Purchase Order throughout the period of performance. Please price accordingly.

For example, is the CLIN above asking for 2,688 bottles of water distributed in 24-bottle cases (112 cases), or 2,688 cases of water (64,512 bottles of water total)?

The Unit Quantity refers to the number of CASES per the description, not bottles. The only single bottle requirement is for the 5-gal units.

For example, Line 002 in the description states 16.9oz bottles (24 bottle cases) and the UOM for bidding is set to "EA." does EA mean 2688 cases? or does EA mean 2688 16.9oz bottles? Same with Line 003 1 gallon bottles (6 per case) with a UOM of EA, does EA mean 6 gallon case or 32 individual gallons? etc etc

Yes, EA refers to the description of the requirement. If the description is for 16.9oz bottles (24 bottles case), then EA refers to each CASE, and the requirement is for 2688 CASES to be provided for each MONTH. As noted in the SOW, how this water gets distributed throughout the month is based on coordination between the Contractor and the Government. But all amounts shall be required during the MONTH. The same for the 1-gallon bottles. EA refers to a "case of 6" or 32 cases. Distro to be determined but all cases shall be required. The only single bottle requirement description is for the 5-gallon bottles. Those are listed "per each 5-gallon bottle" only.
The amounts listed are for MONTHLY requirements.
