1. Determine if the entire road is scarified. If the answer is yes, please indicate the thickness. 2. Determine if patches are scarified. If the answer is yes, please indicate the thickness. 3. If questions 1 and 2 are answered no, please notify the thickness of the resurfacing. 4. Notify the type of material to be used (fine, coarse or other asphalt).
