1. Is the contractor expected to follow up with case management in reference to Exhibit 1 5.4 Management of Profiles. 2. Is the contracted Providers expected to enter into E Profile? 3. Will the contractor be expected to scrub since we now do not have access unless in a government facility? 4. Will the contractor be expected to enter into MWDE? 5. Will the customer provider Record Recorders or is the contractor expected to provide them? 6. If the contractor provides the Record Recorders, will the customer provide the computer and VPN internet access? 7. Will the customer be providing Behavioral Health Specialist? 8. Will the government supply the provider computers and printers? If so, how many. 9. May we do dental treatment in the armory if privacy screens are utilized? 10. Will the customer provide the refrigeration for the immunizations at all locations? 11. Will the customer allow us to plug in a hearing trailer? 12. The added Fayetteville may have been filled out incorrectly on the dental treatment hours. Are you stating there are to be 149 Total Treatment hours or Number of SM? The rest of the requests were sent out in hours and based on 30% of the total number of SM attending. Can the customer please clarify?

Due to the power that hearing trailers take with the power that the dental equipment takes, we recommend the contractor brings a generator so breakers are not tripped 12. The Fayetteville event is different than others as we are focusing on fixing soldiers with deficiencies. It is projected that with the current number of DRC3s and the projected number of DRCs that there will be 149 soldiers that will need treatment completed at the event.

1. Will this be issued as Lowest Price Technically Acceptable or Best Value? 2. Is this a single or multiple award? 3. Will the government issue CAC cards? 4. Will immunization serums be provided by the NCARNG or will the contractor be responsible? 5. Will the NCARNG provide DNA cards? 6. In reference, to Optometry services for the MOB Events, will the soldiers require RX services and glasses with fittings? Or only RX services? 7. Will POST or LIVE MEDPROS be required for any medical services? If post, would it be 5 days after an event? 8. Will case management be required for dental treatment services by the contractors? 9. Will eProfile or eCase be required of the medical providers or will the NCARNG issue profiles/consults? 10. Will behavioral health be required at any event by the contractor? 11. Are we required to bid on all requested events for this contract? 12. Per our past experience, most of the armories can have all medical and dental services completed inside (on drill floor). Can the events be confirmed that the same is available as this could save the government on cost?
